29 CFR 1910.28(b)(13)(ii): When work was performed at least 6 feet (1.6 m) but less than 15 feet (4.6 m) from the roof edge, the employer did not ensure each employee was protected from falling by using a guardrail system, safety net system, travel restraint system, or personal fall arrest system: (a) Liberty Oilfield Services Inc., at 9540 E 104th Avenue, Henderson, CO: On 9/8/2020, the employer did not ensure that employees were protected from falling when working upon the roof of an interior structure. Equipment, including quality control and laboratory equipment, was stored upon the flat roof of the laboratory. Employees were directed to retrieve equipment from the roof. A guardrail system was not in place. Employees were not protected by a restraint system or fall arrest system. This condition exposed two employees to an approximate ten foot fall hazard.
