29 CFR 1910.184(d):Each day before being used, the sling and all fastenings and attachments were not inspected for damage or defects by a competent person designated by the employer: 1) On September 6, 2020, the employer did not ensure that slings and attachments used in rigging to hoist a grain elevator belt were inspected by a competent person. The materials used for rigging failed, which caused the grain elevator belt to fall and strike three employees. Two of the employees sustained injuries that required hospitalization.
