Tri-State Garden Supply, Inc dba Gardenscape, Inc., On or about September 2, 2020, at the address of U856 State Route 66, Archbold, Ohio 43502: In the Production area, the employer did not ensure employees were protected from falls while performing daily cleaning operations of hopper on Hamer Bagger Line # 1. Employees performing these activities were exposed to a fall of approximately 9 feet to a concrete surface below.
