29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service: a) On or about August 28, 2020, at Dock #8, Door #15; employees transported welding equipment and other materials with a Toyota industrial forklift truck (Forklift #11) that was not inspected before being placed into service. The forklift did not have an operable horn, functioning rear lights, and an operable reverse audible alarm.
