29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator: a) Clearview Operating Co., LLC dba The Grand Rehabilitation and Nursing, Queens, 157-15 19th Avenue, Whitestone, New York: On or about April 8, 2020, and continuing thereafter, the employer did not complete respirator fit testing for employees (including the Certified Nursing Assistants (CNAs) and the Rehab CNA) required to wear N95 respirators while providing treatment COVID-19 positive residents and persons under investigation (PUI) for COVID-19.
