29 CFR 1910.134(f)(2): The employer did not ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter. a) 874 Rt 70E, Marlton, NJ: Employees such as Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) who were provided with, and required to use, tight-fitting respirators were not provided fit test. The employees provided direct care to patients with confirmed and/or suspect COVID-19. Violation occurred on or about 5/6/2020 and thereafter.
