29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: (a)Vision Builders, Inc., at 1201 East Cliff Place, Sioux Falls, SD 57108: On or about and at times prior to June 19, 2020, the employer did not ensure employees were properly trained in the recognition and avoidance of unsafe conditions applicable to their work environment. The employer did not effectively instruct employees in identifying hazards associated with commercial construction to include, fall protection, ladder safety, as well as PIT operation regarding framing activities on a commercial project.
