29 CFR 1910.134(f)(2): The employer did not ensure that an employee using a tight-fitting respirator was fit tested prior to initial use of the respirator, whenever a different respirator facepiece was used, and at least annually thereafter: a) Community First Healthcare of Illinois, Inc. - Intensive Care Unit (ICU) 5N/5S located at 5645 W. Addison Street, Chicago, IL, 60634: On or about June 9, 2020 through July 28, 2020, the employer did not provide a fit test to employees required to wear respirators whenever a different respiratory facepiece (size, style, model or make) was used. The employer required employees to wear respirators including Halyard N95, Gerson N95 and 3M 6300 half-facepiece elastomeric respirators to protect against the SARS-CoV-2 virus while providing care to suspected and confirmed positive COVID-19 patients. Employees(s) were not fit tested for all types of respirators they used in the workplace. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
