29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: (a)C & C Construction, Inc. at 82 Burlington Northern Santa Fe Railway, Hazel SD 57242: On or about and at times prior to May 22, 2020, the employer did not ensure that employees were properly instructed in the recognition and avoidance of unsafe conditions. Employees were not instructed in the recognition and avoidance of unsafe conditions regarding a material handling hoist. The hoist was used to lift personnel into position to perform welding activities on the existing tower section. This condition exposed employees to the hazards related to the construction-related activities throughout the site.
