29 CFR 1926.416(a)(3): Before work began, the employer did not ascertain by inquiry, direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work could bring any person, tool, or machine into pysical or electrical contact with the electric power circuit: a) At the jobsite, on or about 5/15/20: Employees were not protected from electric shock while working in a cabinet that had live parts operating at 760 volts DC. The circuits in the cabinet were not tested prior to work, to determine the hazard that resulted in an electrocution. Employees were performing tasks including, but not limited to, cutting a bus bar, feeding and landing cable inside the cabinet.
