ESTABLISHMENT: On or about April 7, 2020, the employer did not provide a medical evaluation to determine the employees ability to use a respirator before the employees were required to wear KN-95 filtering face piece respirators during work tasks such as (but not limited to) providing patient care for residents who were COVID-19 positive or were suspected of having COVID-19.
