29 CFR 1910.146(d)(4)(vii): Under the permit-required confined space program, required by 29 CFR 1910.146(c)(4), the employer did not provide, maintain, and ensure the use of equipment, such as ladders, needed for safe ingress and egress, by authorized entrant: A) Morral Companies, LLC, located at 132 Postle Avenue, Morral, OH 43337: On or about April 16 and April 17, 2020, the employer did not ensure that a ladder was used for safe ingress and egress from permit spaces such as the Lawn Mixer located in the Liquid Department. Employees, who conducted cleaning/unjamming and maintenance activities inside the Lawn Mixer, used gussets/braces inside the mixer during the ingress and egress. Employees were exposed to fall hazards, as well as thermal, atmospheric, engulfment, and mechanical hazards.
