29 CFR 1910.147(c)(4)(ii)(B): The employer had not developed, documented and utilized specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: a) PB-15 "Dino" Svartek Grinder: On or about April 7, 2020 and at times prior thereto the documented procedures for the control of hazardous energy on the PB-15 "Dino" grinder did not identify the shovel cylinder and guide rods as other required machinery that must be locked out before beginning maintenance on the grinder. Note) Certification of corrective action is required for this item.
