29 CFR 1910.147(f)(3)(i): When servicing and/or maintenance is performed by a crew, craft, department or other group, they did not utilize a procedure which affords the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device: a) Hoppers for the dust collector systems: on or about March 30, 2020 - In dust collector system hoppers, employees were exposed to a crush-by hazard while performing maintenance in dust collector hoppers when the employer failed to implement the group tagout personal accountability procedure by having each employee sign-on and sign-off the Master Red Tag form before restarting a dust hopper.
