29 CFR 1910.132(c): All personal protective equipment was not of safe design and construction for the work performed: a) Haas CNC Lathe, Model DS-30Y - Employees were exposed to entanglement hazards while donning thick material handling gloves which were not of safe design or construction for manual work performed next to the rotating chuck and work piece. While hand polishing a work piece inside a Haas CNC Lathe, an employee sustained an amputation injury when one of the gloves became entangled in the rotating parts, on or about January 22, 2020. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
