29 CFR 1910.147(c)(4)(i): The employer did not develop, document, and utilize procedures to control potentially hazardous energy while servicing and/or maintaining the equipment. On or around January 15, 2020 at the Phillips 66 Sweeny Refinery in Old Ocean, Texas the employer did not develop, document and utilize procedures to control potentially hazardous energy when employees opened vents on equipment in the Main Fractionator and Overhead System while steam flow in the system was ongoing and exposed workers to hazards that cause thermal burns, respiratory injury, and dermal injuries. a) The employer did not develop, document, and utilize procedures to control hazardous energy when workers opened the 95-200 Main Fractionator Tower vent. b) The employer did not develop, document, and utilize procedures to control hazardous energy when workers opened the Fractionator Overhead System 95-12 Reflux Drum vent. c) The employer did not develop, document, and utilize procedures to control hazardous energy when workers opened the Fractionator Overhead System 95-31 OH Accumulator vent.
