29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about January 23, 2020, written machine specific energy control procedures were not utilized to insure that the 136 converting machine was deenergized and locked out when employee(s) were engaged in machine set-up, and blade maintenance as part of the normal tasks associated with servicing of the machine. This exposed employee(s) working on or around the converting machine to point of operation, ingoing nip point, and rotating parts which can result in struck by, crushing, and/ or amputation injury. Converted Products Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i) , which was contained in OSHA inspection number #1134768, citation number 1, item number 1, and was affirmed as a final order on August 17, 2016, with respect to a workplace located at 8501 West Tower Avenue, Milwaukee, WI. 53224.
