29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). a. The Pullman Company dba Tenneco, Inc. located in Napoleon, OH: On or about January 14, 2020 the employer did not ensure an employee who was performing maintenance on the DA Tank drain to remove an obstruction, properly operated the isolation valve in a manner that prevented the pressurized hot water from exiting the DA Tank. The employee was injured when the pipe blockage cleared and exposed the employee to 220 degree water.
