29 CFR 1926.1060(a)(1): The employer did not provide a training program for each employee using ladders as necessary, enabling each employee to recognize hazards related to ladders and the procedures to be followed to minimize these hazards: a. On or about December 20, 2019, at the jobsite, the extension ladder positioned as a means of egress, for untrained employees working in a trench, which was 9 feet 4 inches deep, with vertical sides, and in previously disturbed class C soil, was positioned 40 feet from the employees.
