29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: a) On or about November 18, 2019, in the loading area of the Calumet Products facility, employees were exposed to injuries to the head from falling objects when detaching and moving the loading arm from a vapor line.
