29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a.) On or about November 19, 2019, the employer did not ensure employees were protected while performing service and maintenance activities on Krones Labeler #1 (Model: Starmatic, Serial # K082489). Employees were required to change over activities on Labeler #1 and did not place a lock on the disconnect switch, exposing them to crush hazards.
