Co., Inc. jobsite located at 539 South Main Street, Findlay, OH: The employer did not ensure employees performing electrical work on the third floor of the MPLX building de-energized the lighting circuit prior to performing the work. Employees de-energized the circuit prior to disconnecting the bank of lights but after the lights were disconnected from the main feed, they re-energized the circuit. On November 6, 2019 an employee was attaching the wires from a recently installed bank of lights to the energized feed wires inside of a junction box and suffered a severe electric shock.
