29 CFR 1910.147(f)(1)(ii): Employees were not removed from the machine or equipment area in accordance with paragraph (e)(2) of this section during testing or positioning of machines, equipment or components thereof: a. Extrusion Line 3 - An employee suffered an amputation of two fingers while not removed from the machine hazard area when the rollers of the extrusion machine were lowered onto his hand while he was manually feeding the plastic through the rollers.
