On or about November 3, 2019, and at-times prior thereto, the 2017 reactive chemicals process hazard analysis for the Dow Chemical Company Glycol #2 facility in Plaquemine, Louisiana did not identify, evaluate and control the hazards presented from reverse flow of sulfuric acid water solution from the acid scrubber (T-410) to the Knockout Drum (D-410), in the presence of liquid Ethylene Oxide.
