29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a. Site- On or about November 6, 2019 an employee using personal fall arrest equipment while working from the roof fell approximately 21 feet where their personal fall arrest system was not installed to prevent the employee from free falling more than six feet and contacting the ground. b. Site- On or about November 7, 2019 an employee using personal fall arrest equipment while working from the roof was not protected from a fall hazard where their personal fall arrest system was not installed to prevent them from free falling more than six feet.
