29 CFR 1910.145(c)(2)(ii): Employees were not instructed that caution signs indicate a possible hazard against which proper precautions should be taken: On or about and times prior to October 30, 2019 the employer did not ensure that employee(s) were instructed of the purpose and potential hazards against which proper precautions need to be taken when working with equipment such as, but not limited to, rotary valve "cyclone" machines. This condition exposed employees to caught-in-between hazards.
