29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: a) Warehouse - On or about August 30, 2019, the employer did not mark the yellow Hyster forklift, model #S80XM, serial #F004V03020C, to identify the blue boom attachment and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. No abatement certification or documentation is required for this item. b) Warehouse - On or about February 3, 2020, the employer did not mark the Raymond forklift, model #750-DR32TT, serial #750-14-BB41727, to identify the yellow carpet boom attachment and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered.
