29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a. At the transfer cart area, on or about 10/10/2019: Employees were performing maintenance on a transfer cart which has electrical, hydraulic and mechanical energy sources. The employees did not affix lockout or tagout devices to any energy isolation devices which allowed the unexpected start up or the machine.
