29 CFR 1926.850(e): The employer did not determine if any type of hazardous chemicals, gases, explosives, flammable materials, or similarly dangerous substances were used in any pipes, tanks, or other equipment on the property. When the presence of any such substances was apparent or suspected, the employer did not perform testing and purging to eliminate the hazard before demolition was started: (a) On or about September 19, 2019; an employee was performing demolition operations in the Consumer Plant and were cutting and draining a steel pipe which contained Univar Ammonium Hydroxide. The employer did perform testing and purging to determine if any hazardous chemical was in the pipes or tanks prior to beginning demolition work. This caused the chemical release from the pipe. An employee was exposed to severe chemical skin burns and eye damage hazards. (b) On or about September 25, 2019; employees were performing demolition operations in the Consumer Plant and were cutting and draining a steel pipe which contained Troy Chemical Corp. Polyphase AF1. The employer did perform testing and purging to determine if any hazardous chemical was in the pipes or tanks prior to beginning demolition work. This caused the chemical release from the pipe. Employees were exposed to severe chemical skin burns, eye damage, and respiratory inhalation hazards.
