29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): A) Diamond Products, Limited, located at 333 Prospect Street, Elyria, OH 44035: On or about July 15, 2019, the employer did not ensure that all the steps outlined in 29 CFR 1910.147(d), required for the control of the hazardous energy sources, were performed. Employees did not deenergize the Dr. Fritsch Cold Press # 5 (Hydraulic Unit, Type KP 35, Number KP 604), located in the Cold Press Department, prior to conducting activities such as, but not limited to, changing dies. Employees were exposed to caught-in-between hazards that could result in injuries while all hazardous energy sources were not controlled prior to performing the tasks.
