29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: (a) Brand X Hydrovac Services, Inc. at Stapleton S Dr and Airlawn Rd, Denver, CO: On and around August 29, 2019, the employer permitted employees to work in proximity to an energized powerline with the boom of a vacuum truck but the employees were not protected against electric shock by a safe working distance or by the de-energizing and grounding of the powerline. This condition resulted in the fatality of a Swamper, who was electrocuted, when the boom of the vacuum truck made contact with an energized powerline, while the boom was positioned by the truck operator with a remote control.
