29 CFR 1910.23(b)(9): Ladders were not inspected before the initial use in each work shift, or more frequently as necessary, to identify any visible defects that could cause injury: (a) Press Department - On or about August 21, 2019 the employer did not ensure that step ladders, used for access to the presses, were inspected before the press operators used them to complete die set-up operations exposing the employees to a fall hazard when using defective step ladders.
