29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards: a) Jobsite located at 1006 North 5th Avenue Philadelphia, PA.: Employees were exposed to electrical shock hazards when carrying items such as but not limited to a 10 foot aluminum flashing adjacent to a 13.2kV live power line without training to mitigate the hazard. Documented to occur on or about 15 AUG 2019.
