29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: On or about 8/13/19, the employer failed to ensure that the employees tasked with operating the forklifts in the yard were provided refresher training every three (3) years. Forklifts including but not limited to the following: a) Hyster H80FT, Serial #S005V01755L, b) John Deere H61 PIN *DW444HX589486*, and c) Komatsu WA 200-5 Serial #65620.
