29 CFR 1910.269(d)(6): The established procedures for the application of energy control (the lockout or tagout procedures) did not include the following elements and actions, and these procedures were not performed in the following sequence: On or about July 29, 2019, employees were exposed to electrical hazards while performing maintenance activities on a mobile substation that had been modified by the addition of a maintenance breaker. The employer failed to implement energy control application steps as all energy isolating devices needed to control the energy to the mobile substation were not operated in such a manner to isolate the equipment from the energy source. As a result, the remaining energy control elements involving lockout or tagout devices [(d)(6)(iv)], dissipation of residual or stored energy [(d)(6)(v)], verification of isolation [(d)(6)(vi), were not implemented to protect the employees from electrical hazards during the maintenance activities.
