29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: A) Carbo Forge, Inc., located at 150 State Route 523, Fremont, OH 43420: On or about June 17, 2019, the employer did not adequately train employees to lockout/tagout equipment such as, but not limited to, the 530 Ton Hydraulic Shear (Advanced Machine Design Company, Model # 530-2230; Serial # 9102) located in the Shear Building. Employees did not control all hazardous energy sources such as, but not limited to, gravitational before changing blades. Employees were exposed to struck-by and pinch point hazards that could result in in amputations.
