29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: At a jobsite located at the intersection of Blue Jay Drive and West Kansas Street, Liberty, Missouri 64068, employees were exposed to serious struck by and or engulfment hazards. A stairway, ladder, ramp or other means of egress was not located in the excavation. The excavation was measured at 5 feet 4 inches deep ,4 feet 10 inches deep, and 6 feet 9 inches where the employee was working.
