29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control: On or about May 30, 2019, employees perform maintenance and servicing on various equipment. Authorized employees were exposed to struck by, caught in and amputation hazards in that: a) TP 12 - The procedures do not included a verification step to ensure that energy sources are isolated used by authorized maintenance employees.
