29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: a) On or about May 24, 2019, the employer did not provide effective training to each authorized employee who performed service and maintenance. A Shift Lead Technician did not lockout the Cincinnati Milacron Industries, Product Number VH-500-54, Serial Number 3905A21/92-4, Molding Machine while attempting to fix a wheel on a safety interlocked door and was injured. Shift Lead Technicians were not trained in lockout and were thereby exposed to hazards associated with moving parts and the unexpected start-up of the machine.
