29 CFR 1910.178(l)(3)(ii)(A): Powered industrial truck operators did not receive initial training in surface conditions where the vehicle would be operated that could affect safe operation: (a) WAREHOUSE AREA - Employee(s) used the Komatsu FG18SHT-20 Type D forklift (SN 675473A) to unload carpeting and carpet padding from delivery trucks, and the employer had not trained the operator(s) to secure the trucks that were being offloaded from unwanted movement by using chock blocks or similar methods, on or about May 1, 2019. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
