29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): A) Pexco Packaging Corporation, located at 795 Berdan Avenue, Toledo, OH 43610: On or about May 1, 2019, the employer did not ensure that all the steps outlined in 29 CFR 1910.147(d), required for the control of the hazardous energy sources, were performed. Employees did not deenergize the FMC Flexographic Press # 1 (Model 586, Serial # 3646), located in the printing department, prior to conducting activities such as, but not limited to, conducting roll change-overs. Employees were exposed to caught-in hazards that could result in personal injuries while all hazardous energy sources were not controlled prior to performing the tasks.
