29 CFR 1910.147(c)(7)(iii)(B): Additional retraining was not provided whenever a periodic inspection under 29 CFR 1910.147(c)(6) revealed that there were deviations from or inadequacies in the employee's knowledge or use of the energy control procedures: Employees were exposed to amputation hazards. An employee did not utilize lockout tagout procedures when a conveyor was jammed and had not been retrained in lockout tagout . On or about April 23, 2019, an employee received an amputation injury of their thumb as the employee unjammed the drop down belt conveyor with a crowbar without locking and tagging it out.
