29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) Throughout facility - On April 12, 2019, and times prior to, the employer failed to ensure that at least an annual inspection was conducted on the energy-control system by an authorized employee to verify that procedures were being followed and requirements of the standard are being met, thus exposing employees to hazards not limited to amputations and possible death.
