29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces were inspected regularly and as necessary, and maintained in a safe condition; (a) LifeScape at 2501 West 26th Street, Sioux Falls, SD 57105: On or about and at times prior to April 3, 3019 the employer did not inspect the window well cover on the north side of the main entrance to ensure that it was maintained in a safe condition. An employee suffered a fall of approximately 7.5 feet when exposed to this hazard resulting in fractured vertebra and ribs. This condition exposed employees to fall hazards.
