Text For Citation: 01 Item/Group: 001B Hazard:
29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a. On or about March 26, 2019, in the extrusion department, the employer failed to ensure that an employee who was repairing the container loader on the 1675 Wean United Youngstown Foundry & Machine aluminum extrusion press (press 2; model: 7"-1675 ton press; S/N: 38876) had the knowledge of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy; perform an orderly shutdown by completely shutting down the 1675 extrusion press; isolate the 1675 extrusion press from the energy sources; affix lockout/tagout devices to each energy isolating device; relieve, disconnect, restrain, and otherwise render safe all potentially hazardous stored or residual energy for the 1675 extrusion press; and verify that isolation and deenergization of the 1675 extrusion press had been accomplished, thereby exposing the employee to a struck-by and caught-in hazard. On or about March 26, 2019, an employee was hospitalized while performing the aforementioned activity due to injuries to the left leg when it was caught in the stem loader of the 1675 aluminum extrusion press.
