2-3 29 CFR 1910.28(b)(3)(ii): The employer did not ensure each employee is protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems: (a) On March 20, 2019, two employees working in the plumbing test department were exposed to a stepping-into hazard in that the sump pump had been placed down into its pit and no cover was installed over it leaving a hole approximately 9.5 inches wide by 16 inches long and 12 inches deep.
