29 CFR 1910.147(f)(3)(ii): Group lockout or tagout devices were not used in accordance with procedures required by 29 CFR 1910.147(c)(4): A.) U.S. Bronze Foundry and Machine, Inc., 18649 Brake Shoe Road, Meadville, PA 16335, on or about February 18, 2019 and times prior thereto: Employees were performing service and maintenance work on the Bullard 66" Cutmaster mill (machine #1868). The authorized employees did not affix energy isolating devices to the electrical disconnect.
