29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): On or about January 22, 2019 and at times prior thereto, maintenance employees who worked on machines and equipment were exposed to caught in, struck by, amputation and electric shock hazards where: a) the employer did not ensure each energy control procedure was clear and specific with steps to shut down, isolate, block, and secure all hazardous energy; and b) the employer did not ensure each energy control procedure included steps of the placement, removal and transfer of the lockout devices and who was responsible; c) the employer did not enforce compliance of the use of specific procedural steps to eliminate all energy sources and residual energy sources prior to performing maintenance on the machines or equipment.
