29 CFR 1910.1030(f)(2)(iv): The employer did not ensure that employees who declined to accept the hepatitis B vaccination offered by the employer signed the statement in appendix A: Workplace, Newark Liberty International Airport, Newark, NJ a) Two Crew Leads, One Supervisor and One Manager authorized to clean up blood or blood contaminated materials and occupationally exposed to blood had been working for approximately 4 months to 4 years without signing a declination form until 2-19-19. Violation observed on or about 4-4-19 b) Two Supervisors, and 8 Crew Leads, were indicated on documentation from the employer that they refused to sign a declination form to consider the option of having a Hepatitis B vaccination at a later time. However, such workers never signed a respective declination form. Violation observed on or about 4-4-19 c) Four Operations Managers, 1 Supervisor, and 15 Crew Leads were not indicated by the employer as having been offered the Hepatitis B vaccination, and did not sign a declination form.
