29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: On or about January 15, 2019, in the combustor cell area, located at 3373 NW 107th Street, Miami, Florida 33167, the employer had not provided training on LOTO procedures for the 42" Bullard, vertical turret lathe while the employee cleaned turnings from the turntable, exposing the employee to lacerations and broken bones.
